Save yourself time at your first appointment by filling out your paperwork at home. Download the forms below, complete them before your visit, and bring them along with your insurance card. Your first visit will include an initial exam, a discussion of treatment options based on your needs and goals, and if needed, orthodontic records can be taken the same day. Insurance
We work with most major orthodontic insurance plans and handle the claims process for you. Bring your insurance card to your first visit and we'll verify your benefits and explain exactly what's covered before any treatment begins. Our team will make sure you get the maximum benefit your plan allows.
Flexible Payment Plans
Your smile shouldn't have to wait because of cost. We offer interest-free monthly installment plans billed directly to your Visa, MasterCard, Discover, or American Express. Monthly ACH debits from your bank account are also available. Pay in full at the start of treatment and receive a 3% discount.
HSA & FSA Accepted
Orthodontic treatment is an eligible expense under most Health Savings Accounts and Flexible Spending Accounts. Using pre-tax dollars can significantly reduce your out-of-pocket cost. We'll help you understand how to apply your benefits.

How do I know which treatment is right for me?

What are LightForce 3D-printed braces?
LightForce brackets are custom-manufactured ceramic braces designed to fit each individual tooth using 3D printing technology. They're more precise and comfortable than traditional brackets and can reduce overall treatment time by up to 40%.
What's the difference between Invisalign and Invisalign Teen?
Both use clear removable aligners, but the teen version is designed for growing mouths. It includes compliance indicators so parents can confirm their teen is wearing them enough, plus six free replacement aligners in case any get lost.
Can I get braces if I've already had orthodontic treatment?
Most patients experience mild pressure or tightness for the first day or two after switching to a new set of trays. That's a sign your teeth are moving. It's significantly more comfortable than traditional braces since there are no wires or brackets to irritate your mouth.
Do you offer same-day retainers?
Yes! When your braces come off, we fabricate your retainers the same day so your teeth are protected immediately. No waiting a week with nothing holding your results in place.
What if my child isn't ready for braces yet?
That's perfectly normal! The first screening at age 7 doesn't always mean treatment starts right away. Dr. Miller will monitor your child's growth and development over time and recommend the ideal window to begin treatment when it will be most effective.
